Yeah, the Masquerade titles seem like the best fit. Other than that, I'm not sure what other games to consider "horror rpg", maybe the old Ravenloft games by SSI? Most rpgs are rather high fantasy unfortunately.

To a certain extent, maybe Planescape Torment could be considered horror, too, but it's no pure horror rpg. Arx Fatalis isn't either, but it has great atmosphere and is therefor quite creepy at times. It's one of the few games that actually managed to scare me (in a delightful way). If you own Neverwinter Nights or Forgotten Realms Unlimited Adventures, you can download a couple of horror and zombie survival campaigns from their respective community sites for free though. For NWN I recommend for example "Revenant", "Vampire: Heaven Defied" or "Excrucio Eternum", but there are lots of other great modules that would fit the description.

If you're into JRPGs, you could also try the Shadow Hearts series or Shin Megami Tensei...
They have some kind of horror setting...
I can recommend the game Veil of Darkness (GOG, where is it!?!?), it's an isometric adventure game with action-based combat and light role-playing elements.

It's not scary, but it has a lot of elements from classic horror movies, like vampires, a werewolf, other creatures, gypsies, old isolated village plagued by a evil vampire lord. Check it out in Mobygames.
Has anyone mentioned the original Blood Omen Legacy of Kain? No one is ever going to be blown away by the graphics but its a beauty RPG with lots of missions, maps, potions, weapons, baddies, goodies, hidden treasure, and it's LOOOONG.

I'd go with System Shock 2.
If you're willing to dabble in console titles, I recommend Koudelka and its sequel, Shadow Hearts. Both somehow manage to take horror tropes you've seen a kajillion times and make them feel effective again. (And, it must be said, both have some incredibly creepy monster designs.)

There were more titles in the Shadow Hearts series, but they strayed away from horror into more standard JRPG fantasy.
If you're not strictly limited to PC games, you can take a look at Shadow Madness (PS1). It's never gotten grand reviews, but I enjoyed it back when I played it (probably 2000-2001).

Horror RPGs are really rare, I think it's because RPGs naturally include leveling up and getting stronger which invalidate the horror, to create fear you need to constantly feel threatened, outmatched by your enemies and surroundings. You don't get that when you can revisit low level enemies after going up several levels. Are you sure you didn't mean to ask for horror AND/OR RPGs?

Still, as some have said, there is VtM: Bloodlines. It is not a horror game, but it has a single level that is definitely horror themed and it shines in that regard. Afterwards you may find some more disturbing imagery here and there that can do the trick. Just know that it's not the focus of the game.

Someone else said System Shock 2, I haven't played it but I know that it is a FPS RPG and I hear that it is also a full on horror game.

If you're open to non-RPGs I just started playing Call of Cthulhu Dark Corners of the Earth and I'm loving it. It's set in the 1920's (and I'm a sucker for that kind of stuff) so far I feel like I'm genuinely investigating a mystery, the town you're in feels definitely wrong somehow and I've already run into a couple of "what the fuck was that?!" moments.

"Bloodnet". Its a RPG/Adventure - Horror/Cyberpunk Mix. Sadly have no idea where to get it; would love to see the voiced CD version on gog.
