Yeah please do, your facsimile seems pretty spot on, and I checked archive.org, they have all the basic text, just use the second to the last entry/recorded spot or the next one before that, but avoid the very last entry as its usually void of content.

EDIT:
OKAY I decided I'm going to archive all the gog.com gamecards, since it seems likely we have people who can help provide the missing gamecards. I'll be taking tons of snapshots to finish this project or atleast get us up to date.

I'll start by posting the Witcher 2 gamecard that is no longer available to view on the site.
