Add another voice for The Witcher. It IS sword-and-sorcery, but it definitely does not deserve to be overlooked.
All talk about VtM:B makes me want to play it, I never did so far..

Baldur's Gate << Frankly, this game is all hype. It's a 2nd edition D&D based game, where the main character is .. well something that should never exist in D&D, a main character. (Sorry, peeve of mine). As with all 2nd edition D&D games it seems firmly planted in the realm of "Kill the player fast and often until they get so pissed off that they go play a superior game." ... which there's a lot better out there.
All you BG fanbois? Go play a good game :p
Icewind Dale << .. it's better than BG in that it actually allow for a custom party built for your style.. But it's still a 2nd edition D&D game, so.. yeah, you'll die a lot.
Deux Ex << never played it.
Fable << Meh, decent game, aging thing pissed me off to no end. Nothing super special about the game though. It's just a mid-grade action oriented Wester RPG.
Arcanum << PLAY IT! Seriously, Steampunk RPG. One of 3 game by Troika, and IMHO their best. It's like what Fallout 3 should have been! >.>
Fallout 1 & 2 << If you've not played these two? What's wrong with you?!
Freedom Force << ... Skippable, but fun if you need a stupid RPG and you like the hero genre. It's a tactical squad based RPG, a major rarity. you control a squad of up to 4 super heroes and SAVE THE WORLD! >.>

Temple of Elemental Evil << If you like D&D based game, play this, It is the closest to a real D&D experience you will find in a video game. The plot isn't great, it's action oriented, but it's mechanically good and lets you build the party to suit your style.
The combat mechanics for D&D combat are top notch, so anyone familiar with D&D 3E will be right at home.
It's also a Troika game.
Of course, play VtM: Bloodlines too, at least if you like Western RPGs. Not a huge fan of them myself, but VTM is still teh awesome. (but then it IS Troika. .. yes, I am a fan of Troika.)

Do yourself a favour and find a copy man, one of the richest RPGs I've ever played. I'm regularly tempted to load it up again just to see what the latest community patch has done

I guess I was just never big on the vampire thing in general. They seem too villain-ish — what with the blood-sucking and unholy rituals — and I've never really had an urge to play a villain. The same reason I haven't played The Force Unleashed yet.

Thats the good thing about VTM, vampires aren't bad guys or good guys, they're just people in a fucked up situation. Several decisions in the game are based on the easy but nasty road that costs you your humanity or the more difficult but humane path that rewards it. You get sinners and saints in equal measure. Actually thats a lie, most of them are sinners but they're not stupid moustache twirlling villains and most of them are doing what they do because they think its the right thing to do
You're also not really a bad guy in TFU either, you might be vader's apprentice but many of the people you kill are nasty pricks anyway

Balder's Gate is a bitch, yes. But it's something to atleast experience, whether you love it or hate it. Likewise for Icewind Dale and Neverwinter Nights.
You might want to check out Deus Ex.. It's essentially an FPS/RPG hybrid with a rich story, and some "choice". The story is probably inclined for the conspiracy types, though.
Fable was a fun, action RPG. I mention it purely because it was slightly different, and thus worth a look at.
Freedom Force, again, not for the conventional RPG "norms" but rather for the feel of the game. 1950's America, super heroes, Cold War villians? Not many videogames have tread that path, I believe.
As for Eastern RPGs.. Well, I have a small selection, but not a huge fan.

Bloodlines is good. But make sure you add the community patch before you even attempt it. So that also means not buying any version that's not compatible with it.
Deus Ex is pure gold. One of my all time favourites.

The Witcher is highly recommended. Great choices and consequences, awesome alchemy system, brutal combat and a badass main character! 100 hours of pure entertainment! :)

Gothic 1,2 - brilliant but underrated game

I played Fable and Freedom Force. In fact, I AM playing Freedom Force after getting it on Steam. It's crazy hard.

I hate to say this, but unless you're a multitasking freak on some really powerful amphetamines (warning: artistic license being used), pause. And pause a lot. Might make battles a lot more manageable.

Give Arx fatalis a try. It's only $5.99 and it is basically the spiritual successor of the Ultima Underworld series.
If you are in the mood for some older, but slightly difficult, RPGs then you can't go wrong with either the Wizardry series, Might and Magic series, or the Realms of Arkania series.
