Baldur's Gate series for the depth of storyline, actual game play, beautiful old-school graphics and superbly detailed setting.
Planescape: Torment for the same things but completely in its own way in every respect.
Wow. No wonder those are the two highest rated games on GoG!

I tried Broken Sword 1 for a few minutes some months ago, and that game looks very immersive in its own right.

To be honest, I think it's really relative. I remember regularly jumping out of my skin in the Catacombs Abyss and Wolf 3D, despite the primitive graphics and lack of any sound.

The condemned series was very creepy, The whole premise of just slowly walking around in dark abandoned places with just a pipe or other various handheld object was great atmosphere.
The combat(well the hand to hand bits) made it very immersive as it did help capture the despiration in the people of them to survive, The plot at times was incredibly disturbing and definately worth a playthough, just to hear the thud of hitting someone in the head with a metal object.

Clive Barker's Undying and I'd have to include all AD&D. Along with Myst, Amnesia and the 7th Guest. In reality I could include a ton more, but I figure I'll list my Sophie's Choice cream de la cream.

Shivers. I remember playing the demo as a kid and it gave me the creeps.
Also, the Shadow of the Beast series on the Amiga. And Mega-Lo-Mania (that character select screen music was so haunting...).

StingingVelvet: Thief, System Shock 2 and Dead Space are the scariest games I have completed. Amnesia, Penumbra and others are probably worse but honestly I can't play them.
As for immersive in general Morrowind is probably the most immersive game of all time for me. I feel like that world was just perfectly presented and realized. I acknowledge the technical limitations, like people standing in place all the time, harm immersion but for me it's more about world design and freedom than AI.
Penumbra is the only game in recent memory that actually scared the shit out of me. As for immersive in general Morrowind is probably the most immersive game of all time for me. I bought Penumbra and it already gave me half of an heart-attack tonight. :D
The controls take some time getting used to and it would be even cooler if the in-game text was spoken but it seems creepy enough. :)
Here's hoping we'll soon get "Clive Barker's Undying" (EA) and/or "Call of Cthulu: Dark Corners of the Earth" (T2?) on GOG. I didn't read much good about the second half of the latter but I'd still buy it.
