Ultima VII: The Black Gate

Runners up would include Baldur's Gate and Morrowind. Some games are just so good it's hard to love one more than another. Speaking of which I really need to mention EverQuest as well where I literally spent years of my life. It was amazing.

SpirlaStairs: Persona 3

It's been 3 years since I played it, I've played dozens of games since then and none of them have come close to Persona 3.
I played Persona 4 first, so it's my favorite game of all time. Persona 3 is in my top five for sure though. I just didn't like the character development as much. It felt like there were some pointless characters that were just there to be cool party members. Like the dog?
Probably Planescape Torment and/or the Mass Effect series. Realms of Arkania holds a special spot in my heart.

But there are, many, many games in the highest tier. Fallout, X-Com, Deus Ex, Quake, The Duke, Doom. Plenty of great games have been made in the last 25 years.
Final Fantasy 9.
System Shock. The first one.

But yeah, there are many more games I consider perfect. It's just that I find SS the perfectest of them all.
Planescape: Torment, hands down. It's got a poor combat system, not enough voice acting, few cutscenes, a Great Wall of Text, substandard AI, shiploads of glitches/bugs and a slight but tangible feel of incompleteness. Not perfect, then. But it still is.
Star Control 2. It had exploration, fun combat mechanic, ability to min/max your ship, great dialogue, and a fun plot.

Runners up: Baldur's Gate 1, Master of Orion 2, Rome: Total War, Fallout: New Vegas (once the bugs were patched out, anyway)
This is one of the hard questions.

Arcanum, DE, VTM - Bloodlines, Dune, Original War and FNV/STALKER CoP.
Deus Ex, closely followed by Baldur's Gate 2 and Thief: The Dark Project.
Morrowind.

Runners-up: Deus Ex, Casltevania: Symphony of the Night, Super Metroid, Final Fantasy VI
I guess I have to pick games based on how many times I've replayed them, or used hours on them overall.

Single-player: Starcraft

Multiplayer: TeamFortress Classic

Albeit, I do feel there are probably much better games than either of the two, even in those I've already played.

No way I can pick one favorite game. Have to toss up between Fallout 1, Master of Orion 2, Baldurs Gate 2, Heroes of Might and Magic 2, and Mechwarrior 2 Mercs.

I'm noticing a lot of 2's in there.
Gothic 1 + 2 Gold
Mass Effect series (1+2+3)
No One Lives Forever (1+2)
Unreal Tournament (original + 2004)
Freelancer
Deus Ex series (1+IW+HR)
The Witcher 1 +2
Alpha Protocol

+many more
Planescape Torment
Baldurs 2
Divine Divinity/Arcanum
This is an easy one.

Dungeon Master.
