As everyone seems to be saying, Old World Blues is worth getting for sure. It's basically an expansion pack, even though it only cost $10 at release.
The other three... not as easy. Honest Hearts is really focused on exploration and is basically just a new canyon to explore. There are missions and a story, but it's really, really basic. If you want to just run around in a new area looking for stuff I would say get it. If that's not why you play these kinds of games, don't.
Dead Money is survival horror, which is weird. It has some of the best dialogue in the entire New Vegas catalog, maybe in gaming, but the actual gameplay is a bit lacking. If you like the idea of scrounging around and hiding for survival though, and like rich and well written dialogue, then I would say get Dead Money.
I have not played Lonesome Road yet, but I hear it is a linear shooter style of DLC, like Anchorage in Fallout 3. I am not enthused.
Side note: the Gun Runner's Arsenal DLC is actually pretty cool. It adds a lot of new weapons, some interesting modifications and challenges to boot. That's easily worth the sale price.
