It's because those sounds of your character dying are hilarious in Quake 2.
I'll have to agree with lowyhong and say that Fallout had great death scenes.
Fallout Tactics also had some nasty sequences when characters died from an automatic gun bursts.
I think the most frustrating for me (as well as darn cool when it first happens) is being shot off your horse in Mount and Blade Fire and Sword. One shot kills from a rifle are amazing - till you're on the receiving end of the darn things!

Oh, man. I had forgotten about those 20 second cut scenes that couldn't be skipped. Probably one of the worst.

Delixe: I'm trying to remember if it was Soul Reaver or Shadowman, one of those two I played on the Dreamcast anyway. When you died you didn't die you went to some kind of limbo where you kill things and gather energy to come back. Must have looked great on paper but it was tedious to play.

Now THAT is a hard luck story....
I just remembered Army Men 2 and Army Men: Toys in Space. Those games had some funny death sequences (particularly Toys in Space). I think Toys in Space was the first time I ever saw a plastic toy pee himself.

Just thought of another:
Dying in a dungeon in EverQuest and having to do a corpse run without all of your equipment. Unless you knew a Necro (or could convince one to help), that was some of the most tense moments of my poor bard's life. Dying in an MMO could be very painful depending on the penalties associated.

I quite quickly got fed up with the death cinematic in Baldur's Gate.

Not that I died often (I don't quite understand those who say the beginning of that game is difficult), and it's not like I forgot to save in case I did die (I had several dozens, if not hundreds, of saves for each playthrough... until I lost them) but I did dislike that animation and I hated its accompanying audio.
Well, Dead Space 1 + 2 death scenes were very satisfying in a gruesome way. I think the death scene in the "eye surgery" machine was one of the only death scenes in the last decade that made me jump in my chair. I only slipped slightly at the end ...

Damn! Beat me to it! ;) Yeah, Dead Space has really nice death animations for every zombiemorph you encounter. It's one of those rare games where I find myself committing suicide just to see what happens.

It took me like 10 tries to get past that eye surgery bit. I guess mouse and keyboard aren't good for everything? I'm NEVER doing remote surgery. ;)
I played terraria for like 4h and then i walked into the jungle,the night came fought alot of monster,but survived(it was very close) after that i went into a cave,found something called shield statue and got overwhelmed by monsters,died horribly and lost all my stuff
Getting sacrificed by a Kobold in ADOM :D

Never Again medium(!) difficulty...
Something called Mega Man 3. I got it with a bunch of games from my uncle [it later came out, he took his son's games because he though he 'grew out' of them, i'm happy my parents are not like my uncle] including Space Quest 1-3. Ah..
Anyway, MM3 had a very irritating thing about it, everytime the character was hit, he'd fly back a bit, usually falling from a very high tower, which i spent 3 hours trying to climb. And then, i died, and had to repeat the whole level again. And the character would make this dumbass blinking animation while at it. Argh! Frustrating doesn't come close. This is the first game i've played that made me want to throw the computer out the window.
Also, this is one of the reasons i grew up hating console games that much.
