Every boss battles in Dark Souls 1 & 2. Except when I did some online co-op of course :o)

MM2: Use a party of Clerics so that everyone can use Mass Distortion. Spam Mass Distortion, and when the damage drops below 500, switch to using Proton Blades as items. To survive breath attacks, you can use Max HP potions and Divine Intervention.

Xeen: Have your highest level character cast Day of Protection and Power Shield via item. Then use the Xeen Slayer Sword as your weapon. You can trade it every turn, or you can use the "I LOST IT" cheat to get them for everyone. (Energy resistance would also be good here.)
Yeah, using a party of Clerics in MM2 seems to be the best choice, since you'll likely have a full roster with all the NPCs.
But for Xeen it would require some meta gaming.
Never did a serious attempt at the Mega Dragons before, since I though they were unkillable. I should have known better.
Lord Brevon in Freedom Planet. God damn he's a relentless bastard.
Sega Genesis. Stargate. Final Boss, Ra. Never did i beat that on console, never did i beat that on emulation.

Darksiders 2. Argul, but only if you accidentally wake him up, while being underlevelled. Or Absalom in the arena, on apocalypse difficulty, should you challenge him unprepared and underlevelled. Beliar, only because his final shield is too tricky and require speed and precision.

Blade of Darkness. The first Vampire boss can be a real, big pain. Same applies to semi boss enemies, minotaurs with stone hammers, especially when you fist start encountering them.

Vampire Bloodlines. Ming Xiao. Challenging her with anything else other than a fully maxed Thaumaturgy Tremere can be very perplexing and frustrating for you. Vampire Redemption, dark ages, Nosferatu Sewers, Othelio, the boss wielding the berserk axe, or Mercurio, pre-patch era, the first boss on Cappadocian monastery.

Castlevania Symphony of the Night, that boss which is said to be Drac 's sworn enemy, giant golden Minotaur with lightning attacks, cannot remember its name, sorry.

Golden Axe, Death Adder in the original arcade, especially if the arcade's difficulty had been raised from settings.

Hmmm... toughest bosses..

There were a couple at the end of Xenoblade Chronicles that I was never able to beat, but I can't remember there names now. I'm sure there will be some more from Xenoblade Chronicles X as well by the time I finish that.

The Master (I think that's what he's called, the vampire boss of Dreadfall) in ToME is an absolute bitch. Mainly because he has minions (and summons more minions) and because he's a vampire he comes back to life after you kill him. I've killed him two or three times, but it was never easy.

Never beat him. Closest I got was about down to 10% in Round 3.


Decades later: latest SF2 version on XB360; never passed the third guy on easiest difficulty :P (damn those aging reflexes)

You are complaining about "meta gaming" being a bad thing when the series has parts that expect you to do that? (Remember the "Element Orb" in MM2?)

As a side note, while playing Wizardry Gaiden 3, I ran into a similar sort of puzzle. Of course, that game doesn't have hirelings, so the solution is different, but still similarly meta-gamey.
I hate boss battles. I'm terrible at them :D I guess that's why I tend to think that most bosses I managed to defeat could not have been all that tough. But let's try:

Star Trek Voyager: Elite Force- I don't remember what the fucker at the end was called, but he was tough as nails. It took me forever to defeat him. I think I was in some kind of berserker rage by the tme I finally did it.

Prince of Persia: Warrior Within- that game was hard all around, and pretty much every boss fight along the way cost me a lot.

Tomator in Lost Vikings- Yeah, I know. Not exactly one of the usually brought up epic boss battles. But for me ot was possibly the most tense. The whole last level is very difficult, so knowing, while fighting Tomator at the very end, that if I fuck up and even one of the vikings dies I'll have to do the whole level again (for the 437 time or something like that) made it feel like really a matter of life and death.

I've heard some people say that Kreya in KotOR II and Lucifer in Painkiller were tough, but I actually don't remember them as so bad.

The ones who defeated me:

Demon Stone- the final boss is just a damage sponge the kinds of which I've rarely seen. Two of the three characters you control in the game (and switch between on the fly) are almost entirely useless in this fight. It just seems to go on forever, with waves and waves of minions in between very short fights with the actuall boss. But what really defeated me was the unskipable cut-scenes. There is one before the fight, and one in the middle (hell they may be even more I never got to), and I just got so sick os watching them I finally gave up.

Dungeons of Dredmor- I only got to Lord Dredmor once, and it wasn't even a close fight. He killed me in two hits. If it was any closer, I'd try again, but as it was, I decided it would be a terrible waste of time.

FTL- Lady Luck doesn't love me, so a game where so much depends on random chance is a tough nut for me. I don;t think it was even possible for me to beat the final boss with the ship, crew and weapons I got. Worst of all, he's the "it's not even my final form" kind of boss. I managed to survive two rounds with him, but eventually I went down with the ship. I felt bad about it too. After all the Star Trek and Firefly etc. I've watched, you'd think I'd be a better captain, right?

Vampire The Masqerade Redemption: Not many people seem to mention this final boss among the tough ones. I guess I just fucked up, but no matter how much I try, and how, I can't even get through the first stage of this fight. Maybe I chose my skills all wrong, maybe I've got the wrong equipment, maybe I just suck (pun not intended). And once I learned that there is whole another stage to this fight, I just gave up. Way, way to much time spent on it already.

And lots, lots of others I might describe in detail later, in case the "I suck at video games" point needs even more clarification :D
Devil May Cry´s Hell And Hell mode makes every encounter a boss battle, you die from every hit, while even the standard enemies take a serious beating, never finished this mode in any DMC game.
I usually suck at boss battles...

hm the demon dude from Baldurs Gate - tales of the sword coast. (Looked it up, Aec'letec)
I didnt know alot about DnD back then and he just insta-killed random characters, took a while to figure out how to prevent that. also the last fight of the game (ToB), i only won because i had that one wish (as if rested)

Kayran/Draug battles in Witcher 2... not too hard once you figure out how to do them, but dammit. especially the kayran fight (playing on harder difficulties usually)

The first boss battle in Deus Ex: Human Revolution... highest difficulty and armed with a prod, thanks guys ;)

The Butcher in Diablo/Duriel in Diablo 2
The final boss of Baldur's Gate 2: Throne of Bhaal. I have only fought against this boss solo, which may be why I found it so difficult. It's been a while, but I remember spamming fireballs, summoning dark planetars, and going slayer-form. I had to throw everything I had at the boss, until I finally, barely made it. Also, more than half of my characters portrait was covered up by all the buff-icons :P

The Highland Ravager in Dragon Age: Inquisition. It's the strongest of the ten high dragons in the game. It would be easy enough if it wasn't cheap and summoned so many minions. It would also stun you a lot. All in all, a very annoying fight. After a while, I just gave up and googled a overkill-build, and killed it in 20 seconds.

Cheogh in Blood. I played on Extra Crispy difficulty, and I've later been told that it was intended for co-op, not singleplayer. Anyway, there is not enough ammo on the level to kill Cheogh. You have to find all the ammo, shoot all of it on him, and then stab him with the pitchfork for several minutes. The pitchfork-part is very tricky, of course. You can also crouch down, so he won't hit you, but that's cheesing the system :P

The ninja in Double Dragon 2. That's where most of the extra lives gets wasted, in every run. I've co-op'ed the game with a friend hundreds of times, but we have never found a good way to take out the ninja without losing too many extra lives. In Double Dragon 1, the nemesis was "The Wall" :P
