At least the 15th. The first was test, the second was test_1. Not sure if deleted test threads are also kept in the count.
And do recall that the test threads that were moved to the general_archive are not counted in the url.

Not necessarily, it's simply test_14. It may very well be the second or third test_14.

I'm fairly certain that some threads newer than test_11 have had lower numbers.

JMich: Usual cases for a post not going through are bad tags (missing a closing quote for example), too many characters in the post or trying to post a link with a low rep. Adjust the post you are trying to create accordingly and see if that makes it post.
