Maybe Farcry 3?
Not as in-depth lore nor with heavely predefined characters, but the Mount and Blade series may interest you if you want to write the story the of the characters yourself. If not then try any Icewind Dale or Baldur's Gate, you still have to make a character from zero, but all of them are lore rich and have great predefined characters, specially Minsc in Baldur's Gate 1 and 2.

Kingdom Come: Deliverance is pretty good for that.
70-100 hour playthrough. I don't think there are very many macro decisions, more in how you deal with each situation.

Tyranny is a good playthrough. I think there are several big macro conclusions. I didn't care for the items or combat much, and there isn't really exploring per se. Usually you have an option of zones to go to and do quests. Zones are basically combat, you have to fight towards the objective and that's about it.

Wasteland 2 is pretty involved, lot of good/evil choices, multiple endings.

Bleh, the story is total garbage even before the lone choice at the very end. Dialogue highlights include such gems as, when holding a glowing rock, "This is some magical shit!". Bravo, douche-butt, you should have killed the guy who tasked you with taking it a long time ago, given what he's putting your brother through.

A lower budget "open world" game that does not get mentioned frequently would be "Westerado Double Barreled".

It does feature choices & consequences (both inside and outside of dialog options), different endings and replayability due to different unlockable characters, excluding quest lines and a new villain each time you restart the game!

The game's main plot and gameplay revolves around doing errands for people in order to collect evidences on the identity of the murder of your family, so that you can find and confront him. (His identity consists of various appearance and outfit features, and it is randomly reassigned to a different person among the game world's population.)

both pathfinder games
baulder gate 1 2 and never winter nights 1 and 2 never finished due to the amount of reading
and others i cant rember off hand
Check out Disco Elysium! That would be a great experience for your hands down. One of the greatest RPG of the last decade
with good/evil choices Star Wars Knights of the old republic 1 and 2 are great, not really that open world but they are great classics.
