If I bought the base game from the Apple App Store, can I still buy and install the expansions from GOG? Or is my only option to buy the expansion through the App Store (which is the only choice in-game)? Thanks in advance.

No. GOG won't allow you to get the expansion if you don't have the base game in your library. You can buy it but only as a gift code to gift to others (you won't be able to redeem the code yourself).
