I'm in the middle of my Vampire ; The Masquerade game as a Nosferatu.
This is the singular best (and sexiest) role playing game I've ever experienced.
All good things must come to a pass. When I finish this third go round I would really like to find another similar game that I might get as much enjoyment from.
However I don't plan to abandon Vamp. It has great repeatability. Just don't want to get bored with it.
So, I'm hoping that the GOG forum members would help me in this quest.
Appreciate any and all help.

You could enjoy Morrowind as well. The races are unique and the roleplaying element, albeit minor or trivial somewhat at first sight, very much interesting.
The first Vampire game is out. :P

Assuming in your OP you were talking about Bloodlines, this is the first game in the series though nothing to do with it story wise. It's not a bad game but the controls leave a lot to be desired and it IS NOT "sexy"!! :P

Bloodlines is one hard act to follow.
Was actually just reminded of it again yesterday, and stumbled into this.
Pretty much the only connection it has with Redemption is that they use the same game system / ruleset, taken in a generic sense.
For Bloodlines-like level of writing, probably Planescape: Torment. Other than that, no idea.
For that overall atmosphere, and especially in a not-too-fantasy setting, really no idea.
Same for choices so varied and treated so seriously while still having characters with solid personalities everywhere and a good story to go through.

As Brian Mitsoda put it in an interview I read once, for the team who worked on it the best thing is that they got to work on a game like Bloodlines, and the worst that they most likely never will do so again. (Also, infuriatingly, he was saying there that if he had any say in it there will not be such a game attempted again, because they tried simply too much to be manageable by any team, resulting in the utterly broken release and bad burnout.)

Back to the question, I do like the Gothic 2 atmosphere though. Different, but did feel quite at home whenever I returned to Khorinis over the years.

You could check out System Shock 2, Deus Ex series, The Witcher series is outstanding (but the first game is an overhead view with a unique combat system, that I personally like more than the sequels, but all the games in the series are great), Star Wars Knights of the Old Republic 1 and 2, Fallout 3, 4 and New Vegas . Fallout 1 and 2 are top-down isometric games. Dragon Age Origins, perhaps too much fantasy for your tastes.

Some of these are on sale right now for a few more hours.
Lots of great suggestions above, have you considered Shadow Warrior?
If your main interest is a great story, I'd recommend Planescape:Torment and KOtOR II, in that order.

Reactivity-wise, Dragon Age: Origins is nice, and also has several classes/paths to make for replayability.

Gameplay-wise, BG II has better combat, cool magic system, lots of viable and different combat builds, bigger NPC pool, lots of mods available, best replay value of the bunch, though probably less roleplaying opportunities ingame.

For a more urban setting, Shadowrun: Dragonfall could also work.


EDIT Oh, and I'd avoid Vampire: Redemption, it's a totally different game, and has nothing that sets it apart from plenty of other, better ARPGs.

I have just purchased Planescape:Torment and placed a couple more of your suggestions on my wish list.
Deus X and System Shock, I played when they first came out. I'll play them again through GOG.
KOTOR I had on disk, was lost in my last move, but never played. Will probably purchase again through GOG.
Whitcher looks good. Wondering if the later editions will play on my old rig. Win 7.

Depends on your hardware.
If you want something vaguely in the same gameplay style as VtM:B or Deus Ex, there's E.Y.E: Divine Cybermancy.
Scartch, I'm going to answer your OP in a slightly different way.

When I first bought the disk of VTMB, I played it through four times straight. To date, I've played the game 10 times.

These are the other RPG games which have had the same effect i.e. I've enjoyed them so much I've had to replay them at least 3 times on the trot, then I've returned to revisit several times again over the years:

Septerra Core
Wizardry 8
Gothic 2 Night of the Raven version
Witcher 1
Dragon Age Origins
Baldur's Gate
Divinity 2

I hope you find some games to enjoy from everyone's suggestions.

Surprised to see Septerra Core in that company. Quite liked it, more than I thought I would, but definitely unlike the others.

As for NotR, may have suggested Gothic 2 myself, but the change in character development and the difficulty spike brought by NotR is why I started that game in 2005 and am yet to finish it. Probably not even halfway through last time I gave it a go, which save date says was March 2012. Didn't even dare since, despite the fond memories and quite the draw it has, what I was saying about feeling at home in Khorinis. Probably messed up too, trying to find ways around things that were either just too hard or too frustrating, impossible to do "right".
My eyes and browser's search ability are clearly malfunctioning because I haven't seen Arcanum mentioned yet. Sure, it's fantasy, but it's a weird steampunk type of fantasy by the same developers.
