Can't get achievements in Regalia - Royal Edition.
Did all kinds of stuff: reinstalled, verify/repair, checked firewall, etc. Even reinstalled Windows.
Looked for some people who played this game - looks like for many they worked, at least some time ago.
But there is also one more player who posted on Regalia forum about this problem.

I'm starting to suspect that GOG Galaxy has a general problem with achievements lately.

A Hat in Time
Grand Ages: Medieval
Moon Hunters
Predynastic Egypt
Regalia - Royal Edition
Saints Row IV
The First Tree
The Order of the Thorne - The King's Challenge
Urban Empire

All these games and who knows how many others present problems for achievements with high success rates, they suddenly stop working from today to tomorrow, or you need to restart the game until they work.

I haven't open up the support ticket yet for The Order of the Thorne - The King's Challenge.
At first i had problem with Noctropolis game, achievements weren't unlocking, i had to beat the game 3 times and load God knows how many saves until it started to work. The ticket was opened for 3 weeks until they finally helped me.
Now i'm stuck with Metal Slug X, some achievements got unlocked but others won't no matter what i try.
Some people from my friend list don't have any of the issues i did though.

And yes, this thing that for some there are problems with the achievements while other people have none is very strange. For example, not so long ago I played Noctropolis and Metal Slug X and I had no problem with the achievements.

It should be GalaxyClient.log, support quoted lines for me from that file.

The achievements of Breach & Clear have been completely removed.
In this case in the past they worked, and several people had completed them, I wonder what happens in this case.
If someone had unlocked any of them let me know if they were deleted even retroactively!

The same bug that plagues Dragon's Lair Trilogy and Gekido Kintaro's Revenge is spreading and now for no apparent reason has hit Gwent too.

Are you referring to the icon bug, or the achievements bug?

EDIT: Now that I think about it, considering the thread in which you brought this up :), it has to be the achievements bug... So ignore this.

How long have Saints Row IV achievements not been working? I unlocked one seven days ago but there are other achivevements I have reached the criteria for and not unlocked.

And I got two more games for the list.

Niche - a genetics survival game v1.1.4
"Survivor" for 50 days. I got up to day 75 in Story Mode and it didn't unlock. "Still Alive!" and "Never gonna give you up" are the same kind of achievement for 250 and 1000 days so I assume they are broken, too. I was able to unlock "Sweet Sweet Love" though so it might just be those 3. I have not tried rolling back yet.

Rezrog v1.1.0
It looks like all the achievements are unlockable if you roll back (the vast majority if you don't), but quite a few of them are still broken in various ways. And they are not awarded retroactively.
- "Epic!" and "Legendary!": These are awarded for equipping items of the corresponding rarity in each equipment slot, but they unlocked prematurely for me. I think I already got them after equipping 2 or 3 items.
- "Adept of ___ Damage #": Awarded for defeating enemies with a certain damage type. The damage types are mixed up. I think I got those for Earth damage from dealing physical damage and for dealing Void damage I got both the achievements for Physical and Void damage at the same time.
- "Physical Gem Enthusiast #": Awarded for using physical gems to level up skills. These just don't work at all. I was able to unlock the first one after rolling back to v1.0.8
