Berzerk2k2: I just finished Bright Memory: Infinite on "Revenge" difficulty and NONE of the achievements for this game unlocked on my end.
Another user just complained about the same thing. Finished the game and got none of the achievements. So they are definitely completely broken.

In Ys - Origin the achievement "Truly Gifted" seems to be bugged. I got it unlocked after getting all upgrades/blessings with one character, but you should get it, when upgrading all three characters completely.
Bright Memory Infinite Achievements seem to be fixed with the recent update :)

Thanks for confirming this. Time to do another playthrough.
The sheet says that Surviving Mars is missing 10 achievements, but both Steam and Galaxy list the same 80 achievements. It should be removed from the list.

Ancient-Red-Dragon: For Death end Re;Quest 1, I completed the criteria that should have unlocked the Achievement listed as "III" with the descriptor "Clear Chapter 3."

However, the Achievement failed to unlock for me.

If I close the game and then re-open it and load a saved game that is located in Chapter 4, then this Achievement for having cleared Chapter 3 still will not pop up on my account.

All of the other Achievements that I was supposed to earn seem to have been earned though, including the ones for beating the previous Chapters before 3.
Thanks, I'd wait for at least one more report to make sure it's not a random bug, but I'll definitely keep an eye on it.

Alexim: It's very strange, because I think Of Orcs And Men was released complete on GOG and never received any updates. The achievements were unlocked by a lot of people so they should work.
It's worth trying again by starting over if you feel like it, it may have been an occasional bug.
If your internet connection is not stable, some games on GOG are problematic, and the achievement will no longer be awarded unless you delete all data and start over.

Kevin04: I can confirm the problems. On my main playthrough, I only unlocked about half of the achievements I should have gotten. I even missed many mandatory ones, like "Finish Chapter 3". The triggers aren't downright broken however, as I was able to get 100% after redoing all I missed often enough. There was a fight near the end I failed many times. You're supposed to get an achievement for choosing a dialogue option right before the fight. Only when I retried for the 7th time or so, did the achievement eventually pop.

The way these triggers fail feels very random, couldn't make sense of it at all. The best advice I can give is to try it on a clean run with a very small number of saves, that at least seemed to increase the chances of unlocking. It will work if you really want to get them all.

Lately I've been playing more of both Dragon Star Varnir and Death End re;Quest 1.

With both of those games, on multiple different days, I've had instances of some of the Achievements popping when they should, and other ones not popping when they should.

Two nights ago I beat Chapter 7 of Death End re;Quest 1, and the Achievement for clearing Chapter 7 is supposed pop automatically when you reach that point in the game. Yet for me, it didn't.

Last night when I noticed my Achievement for clearing Chapter 7 was missing, I reloaded a saved game from near the end of Chapter 7, and then I replayed up to the point where the Achievement was supposed to pop, and this time, it did pop.

With Dragon Star Varnir, some of the Achievements also did not pop when they are supposed to at the end of one Chapter/beginning of the next Chapter. But it's possible they would have popped if I had loaded a saved game from those points and tried again (but I didn't have any saved games saved near those points, so I couldn't test that).

And another thing that happened with Death End re;Quest 1 was that the Achievements "Gymnasium of the Mind" and "Switch It Up" and "Call of the Wild" also did not pop for me on the first night when I performed the criteria that should have popped them.

But when I performed that same criteria on another night, then they did pop that time.

So, all of this leads me to wonder if it's possible for Achievements to be pseudo/semi/somewhat broken, whereas they are coded imperfectly, so that they work sometimes, but not always?

In other words, maybe in some cases, there is not always a clear-cut binary division where an Achievement can be neatly classified definitively as either "working" or "not working"...?

Yes, in my experience there are several achievements on GOG that sometimes, apparently without a clear reason, aren't unlocked the first time the requirements are reached, but trying again you can unlock them. See for example Of Orcs And Men, Mark of the Ninja: Remastered, Mary Skelter or Wolfenstein: The New Order in the "Helpful hints" section of OP.

It seems that Dragon Star Varnir and Death End re;Quest could be part of these games that don't have great stability. Maybe GOG's achievement system is not optimized, or these games have some incompatibility that sometimes results in an achievement failing. It would be nice one day to get some clarification from a developer or GOG staff.

Yeah, unfortunately there are these weird bugs on Galaxy that don't seem to have clear patterns, and so an achievement that doesn't work for one person apparently gives no problem to another. In these cases I tend not to add the game to the list right away, because I might clog it up with false cases that only apply to a few people or unfortunate cases.

The case of Hyperdimension Neptunia U: Action Unleashed is emblematic, because I know a friend who has completed at least half of the achievements, and some of them are "common", so they have been unlocked by many players. The other half of the achievements, on the other hand, are "legendary", so maybe they're broken, or they're just very difficult to unlock in a short amount of time.

If you have more first hand information let me know.

Indeed a lot of the game's achievements are still "legendary", so they probably haven't been implemented properly yet. I'll probably have to add the game back to the list if they don't release an update soon.

Ice_Mage: On the one hand, I'm not very optimistic that Deus Ex Revision will be fixed because apparently the issues cropped up with GOG Galaxy 2.0, so nearly 2 and 1/2 years ago. On the other hand, Gemini Rue was released in 2011 and it just got an update a few days ago, so who knows.
I've seen that a suspicious update for Deus Ex Revision has been released, but unfortunately there doesn't seem to be any changes to the achievements.

I read somewhere that there's an update, but I don't know what's in it. I didn't get the blue pip and Updated category in my account. That did happen for two other games recently, so the whole feature isn't broken.

I also don't know if the game somehow silently updated in GOG Galaxy. I can't find the version number anywhere; it's too big to fit in the "More" menu below Additional Executables and gets cut off. According to this post from 2 days ago, the update is just for the offline installer "at the moment" so who even knows.

And yes, the first thing I checked was the achievements lists and there's no change there.
Space Hulk Deathwing Enhanced Edition has 7 fully broken achievements (nobody got them) and 3 more might be bugged only for me.
