I'm so paranoid about quick save and load because of doing this in the past...

Losing savegame data (for any reason).

Losing 100hours of Morrowind back in the day was...traumatic. Tho i did replay the game a few years later in its entirety.

Yesterday playing dishonored I quicksaved when a guard was about to spot me. Less stealth, more facestabbing I guess :P

It sucks when you think you are the only one who screws up like that.
When you're rolling stats for a new character in Baldur's Gate and accidentaly press "reroll" after you have rolled 95. Baldur's Gate (or life) doesn't get any more emotional than that.

Playing an RPG for hours without saving and then: sudden death...

Saving in a situation far above your head / that of your characters without any possibility to escape and not having a save game before that.

Breja: Saving all of your best ammo/spells/potions etc. for when you really, really need them and finishing the game never having used them.
Every game I've played (well, the ones with storable items). I'm a hoarder, and you never know if you're going to need it later, but then find out at the end that you didn't even need it for the boss.

Only exception is Magic & Mayhem 2, where it's really obvious that it's the final level, and you just let rip with the lot (and as a result win easily).
Playing Fallout for hours to find out that I was so irradiated, that I couldn´t make my way back to safety.
I like systems where you get a boost above standard health limits but it slowly counts down to normal. Causes you to be a little more reckless for a moment.
Where is the pain in that?
