This regularly happens with integrated Intel graphic chips. It seems to be a driver issue. When you rename the game's executable to mohaa.exe (= Medal of Honor: Allied Assault), said driver seems to load a profile for Medal of Honor, which also works for Call of Duty.

If you're using an Nvidia card with Optimus technology (automatic switching between an integrated Intel chip and the dedicated Nvidia graphics) you can force the usage of the Nvidia card in the Nvidia Control Panel (Manage 3D Settings => Program Settings. Add the Call of Duty executable(s) and set the preferred graphics processor for each to 'High-performance NVIDIA processor'.)

Thanks for explaining. I will try that.
Isn't there also an issue with vert sync? As in it has to be turned on or off to get the game to work?

is it retail or steam version?
UPDATE: I changed the .exe file name to mohaa.exe it did not work, any other suggestions?

In that case, you have probably run afoul of the game's DRM. One of the later Windows 7 updates introduced a feature that makes it impossible for older disc-based DRM (like Safedisc/SecuROM) to install on your PC. As a result, Call of Duty (which uses Safedisc) will refuse to launch. You can try this workaround and see if it works:

UPDATE: I tried patching the game, it did not work. should I try removing recent security updates?
