Optional permadeath. In Spelunky, at least, it is a built-in facet of the game, not an option (unless the commercial version changed things drastically).

I imagine it's a bit like S.P.A.Z., but with the standard Soldak ARPG elements grafted on? (I only ever got Din's Curse. I kinda feel like Soldak needs to branch out a bit more and try some different things if I'm gonna bother with any future games from them. They're starting to seem a bit like Koei...well, maybe not THAT bad. ;) )

Nah, it's nothing like SPAZ, in my opinion, as the game doesn't have ship physics for starters :)

It's got the standard Soldak elements, yes, but in this game you can have the option to get Drox Guild quests or objectives as well as the regular ones you get during the course of the game :)

Of course but they were there :)
Divinity original sin enhanced edition has honor mode which I think is permadeath. (haven't actually played the game but been researching it)
Pretty sure difficulty options affect whether companions can permeantly die in Expeditions: Conquistador and S2: Silent Storm. Games I'm not sure about are Legends of Eisenwald, Hard West, and Dead State. They all have permadeath, but optional permadeath? Some of the difficulty descriptions can be a bit vague.

Oh, also Pillars of Eternity has a ‘Trial of Iron’ mode, limiting the player to one save that vanishes if the player dies. Wings Remastered might have something similar.
