Well, you didn't mention that you wanted to see an actual 3D model.
That would rule out most first person games to begin with, since in those the light is usually attached to the weapon or sits on the shoulder.
But there are enough of these as well, the probably most famous one being Alan Wake, where the flashlight is a central gameplay element.
Also check the Suicide of Rachel Foster for one with a dynamo.
In at least one japanese survival game you use the cell phone as light source, forgot it's name.
Btw, don't mix up the technology with the gameplay element. Older games usually have a simpler lightning model. So there is a flashlight in them, but what illuminates the room is not a beam coming from it.
