The Lemmings dying by several creative contraptions in the first game and Oh No, More Lemmings. Also, blowing all of them up to create some nice fireworks was quite fun back in the day too.
The pretty horrific deaths of the boy in Heart of Darkness is another.

Many, many deaths in Long Live the Queen (or as I like to call it: How Many Ways can You Kill a Princess?) My favorites: death by chocolate and losing to the enemy king because your decoration stat wasn't high enough.

Classic. That elevator sequence was the very first bit of gameplay I saw of Half Life, when I saw someone playing it in a video game store on a demo station.

Here's a strange one (Wizardry 4):

Wizardry 4, when an enemy kills you by casting MAKANITO early in a combat round, but you (the player) fail to notice it. Then, the rest of the round plays out normally, and it looks like you're doing well with your summoned monsters doing some nice damage to the enemies. Then, at the end of the round, you are unexpectedly hit with the game over screen.

There are lots of other ways to die as well. For instance, as hinted in the manual, there's the effect of getting the amulet without putting on a certain item as well. You could burn up in the flames of hell. You can teleport into rock (but fortunately, unlike in other classic Wizardry, you can reload when that happens). And finally...

You can be executed by Hawkslayer, but not before he orders sushi, eats the sushi, falls asleep fighting you, has tea and brushes his teeth, and fills out a form for you to be sacrificed, and there's nothing you can do about it. (Well, there is of course *something* you can do, but answering that would spoil one of the game's puzzles.)
Killing a giant and getting smashed by his body in Drakan: Order of the Flame. Being thrown by said giant and falling apart was pretty awesome too but I can't find a proper video.

semigroups: Not like a particular character death, but it was so funny after single-handedly killing an army of Nilfgaardians to free Triss Merigold in TW2, as she's walking past all the bodies she's like, you did all this? And Geralt replies, they didn't kill themselves. XD
The actual quote is "They didn't slaughter themselves.", which is as badass as it gets.
It's one of my favourite moments in TW2. =)

Thanks for the correction! Having now just finished the game I have to agree, saving Triss with that last line was one of my favourites as well. :) On a side note, so glad I didn't leave her to Letho, made killing Letho easier. :P

I let him go myself. No idea how that will impact TW3.
But yeah, great moment there.
Which reminds me that I only ever finished Iorveth's path in TW2. I should probably try Roche's one of these days...
