Yeah, neither would I. It's a bit like Donkey Kong in there. But instead of barrels empty wheelchairs are coming at you, there's escalators instead of ladders and instead of a gorilla there's a naked hipster eating feces on top and all you get as a reward when you reach him is a line about homosexuality from a fortune cookie. Also the princess is your mom in a Leia costume. It's a fucked up world.

I would wager it's Hotline Miami but it's not presented from a top-down perspective. And I'll agree with tinyE that the dude does have some severe elephantitis going on :D
