ArthurWalden: Unfortunately, this advice wasn't any use. There were red eyes on pretty much every wall in all directions in the Halls of Vaught. I think the situation is similar to the Arcane Sanctuary. It's in one random direction, but there's no way to know which until you see the button for your quest panel come up which tells you to kill the enemy (the Summoner or Nihlathak) in question.
It has been there since the start. The screenshots in the link unfortunately show D2:R, but I remember farming him ages ago and using the walls to quickly find him.

But yes, Arcane Sanctuary has no way to tell.

Yes, but many locations still follow certain rules when being generated, especially with waypoint placement. Look at the video I linked, the guy goes over the 3 basic layouts you can get of the dungeon before the boss.
And in Nihlathak's case, you can always tell exactly thanks to the visual hint.
