She is the nurse from the movie adaptation of One Flew Over the Cuckoo's Nest. The actresses name is Louise Fletcher.

I looked it up and figured it out, judging by some of her other work she IS a pretty creepy person. :O
One of the only 2 times I did LSD I watched One Flew Over, very bad idea. :/

I preferred him in Five Easy Pieces with its famous diner scene, but Cuckoo's Nest was good too.
