So I told Lars about the Skulz gang's plan to kill Neal and we go there to stop them. I kill all of them but for some reason the guards never join in on my side and then afterwards they turn hostile and start attacking me. What gives?

Sounds like you didn't wait for something to trigger before opening fire.

It's possible I guess. Though once, I tried not even firing at all the whole fight and just let Neal, Ian and Dogmeat handle things. The guards still turned hostile afterwards. Not sure how I can wait longer than that...
