I get stuttering frame rate with the FPS Optimizer, running the game without it runs fine. Changing vsync setting in the gpu doesn't help.

I have only the following patches/mods installed:

Morrowind Code Patch
Morrowind exe Optimizer
4GB Patch
Morrowind FPS Optimizer

Any help is appreciated.

That's really peculiar. Have you tried disabling all graphics improvements like the larger view distance?
Or is 30 fps "stuttering frame rate" for you? I think the FPS Optimizer caps the frame rate at 30 per default. Have you switched that setting off?
You should try tinkering with all settings (there is an advanced settings section) and run it as administrator.

I've never had any issues with the FPS Optimizer before but if you can't get it to run properly maybe you should use MG XE or MGE.
MG XE and MGE don't force you to use their advanced features after all. You can use them to just enable wide-screen resolutions.
(Keep in mind, though, that you have to set the MG XE .exe file and the Morrowind.exe to run as administrator.)

30 fps stuttering. How do I change the cap? I changed the the refresh rate to 60 under the "change Morrowind registry setting" option. Is there another setting for that I didn't see?

Ah, I figured it out and I think I fixed it. The advanced options that are displayed weren't quite clear to me until I read more closely and experimented. Much thanks for your suggestion. :)
