It says it allows an extra item, but as far as I can see, it just takes up my item slot with no other visible effect. What am I missing?

It gives you an extra use of the usable item slot. So if you have a grenade equipped in the usable slot and the rucksack in the other slot, you can throw 2 grenades.
OK, now that makes sense. Still doesn't usually feel worth taking over a gas mask while I'm in Cultist territory, tho. Even with the higher tier +2 use ones I've found since I asked this.

It can be worth pairing up with a medkit or trauma kit in the early game but it's all risk/reward for sure. hope you are enjoying the game.
Yeah, I'm grooving on it. The tradeoffs in inventory options are particularly tough, which is why I was kind of hoping it was supposed to just give me an extra item slot.

one item gives a passive effect (for ex, scope adds to hit chance) and one is usable so it adds an extra button to the character's abilities bar (for ex, grenade can be thrown at an enemy or area - press/click the button to change to the targeting reticle)
