I am playing on the very hard difficulty, aim assist is off.
I am not that MLG pro, but there are few moments. As soon as you get good weapons for headshots, game becomes easy. And if these weapons are tech ones, you can land headshots through walls, facing no danger at all.

Ping your foes, tag them, kill them through walls with headshots, kill them faster if they stick their heads out.
On the other way, how could you fix this, by nerfing everything that works and making enemies into bullet sponges? That will ruin all the fun.

Maybe the solution will be in making enemy AI more aggressive, i.e. charging into player as fast as possible? That will make sniping much more difficult.

Another option - make enemies to move faster, and in less linear way, so you will have harder time to aim at their heads - because now they move steadily, or stay behind covers on the same position. Without introducing "unfair" difficulty it will rise skill requirement for combat snipers.

The last option is to make enemies throw grenades more often. So a sniper won't be able to stay in the same position for long.

Void Eclipse, have you left the starting area yet? Try heading outside of Little China district where you start and it gets a LOT harder out there. Was getting along really well in Little China like you stated usually using a good Revolver but decided to head over to Downtown and Corpo Plaza as a level 14 and got my ass handed to me! I grabbed one of their weapons on a sneak mission and can't even equip it till I hit level 30 which says a bit and the weapons I had, whilst decent in Little China barely made a mark on them in other areas.

Nope, finished the game on very hard. High damage overture revolver and grad sniper rifle are able to oneshot almost everything in the head, bosses are still challenging though.
Yeah definitely the AI needs improvement and I'm finding very hard too easy as well, though I don't want it to be harder in some cheap way.

Pfft. You can get quickhacks that force the target to commit suicide, and they have literally no defence against it. Or even better inflict cyberpsychosis which makes them attack their friends, then commit suicide if there's nobody else around. For the pacifist, there's even one that automatically KO's the target. I can't even remember what guns I have equipped at this point :P

Don't think adjusting the combat AI would make a whole lot of difference given the sheer number of tools at the player's disposal to deal with them. Making the enemy more aggressive might upset snipers, but it'd be incredibly helpful for melee builds.

If it's really something that needs fixing I'd suggest the more effective way would be to make more use of bonus goals in the missions. There's a couple which give you additional loot or money for completing a mission undetected; you could add in similar goals for killing all targets and one for KO'ing all targets. Wouldn't necessarily make the combat any easier, but it'd at least encourage approaches outside of your character's forte.
I think its pretty good. I did body and int with shotgun. Fun so far and definitely not boring. Couple missed shots and you're dead, you need to use dodge too. I play on VH. Valentinos are kicking my ass even Im lvl 39 or so.

I gotta say though I havent used best medi kits, I use the weakest bounce health. Im not sure if the game turns very easy if you go with best medi kits.

Yea the game's AI is absolutely terrible. Enemies need to be smarter AND way more aggressive. When it comes to challenging AI in games, aggression is underrated.
