Should I keep it enabled? I get much better performance with it disabled.

Only if you don't mind screen tearing that appears in the middle of your screen (most of the time) when looking around in the game. V-sync eliminates that tear since it locks/syncs with your monitors/tv max refresh rate. Pick your poison. Do note though, if you enable v-sync it does add input latency.
