I have exactly the same problem. Sound in the menu works, sound while playing works, intro sound doesn´t work.
The audio tracks for the cutscenes are separate from the video files (ie. CRYPTIC.SMK for the video and CRYPTIC.WAV for the audio), so what I'm assuming the game somewhy won't find them.

The movies and their audio tracks are defined in the INI files (BLOOD.INI and CRYPTIC.INI or whatever game you're running) so see that they correspond.

Same problem here, the wierd part is that I had a previous version of Blood (when it was abandonware) that had issues with resolution but the FMV sound worked fine. Did GOG staff changed anything in the sound files?

No. It's most likely that you have a DosBox config issue, as it works fine here.
