rickmum: How did you get it set up that way? I signed up today just to buy Blood and the controls are ridiculous. I can't seem to map buttons to my mouse or change the mouse from moving me forwards and backwards when I move it up and down.
This is frustrating and I have no experience with dosbox so bear with me.
Not too hard, actually. For starters, it has nothing to do with DosBox itself, just to clarify.

You can change most of the controls from the options menu. Editing blood.cfg with your favorite text editor (i.e. Notepad) will also work. Mouse button settings, as far as I can tell, can't be set in game.

Run the setup shortcut in your gog.com start menu shortcuts, and you'll be able to adjust these settings. By default, mouselook is off. Just bind it to some random key, like u, and press it once while playing. From now on, mouselook will be on unless you toggle it back off.

If you did an ordinary install, the game should be installed in 'C:\Program Files\GOG.com\One Unit Whole Blood', but you can right click the game and use 'find target' to locate it. This will actually take you to the game's DosBox directory, just go up a level.

If you ever get upset with your settings, delete blood.cfg and when you run the game, it will generate a new configuration file with the default settings.

If you'd like the settings I'm using, just download the attached image, rename it to "blood.cfg", and put it in the same folder as blood.exe (which, again, should be the One Unit Whole Blood folder). If it asks you if you want to overwrite blood.cfg, that means you're definitely doing it right. Tweak it as you see fit, but it's already configured with modern conventions like wsad and whatnot.

Vagabond: Try installing the OUWB Launcher; it changes the controls to a more modern scheme along with other things.
I still haven't gotten around to trying this myself yet, but this is probably the best option overall, seeing as it has tons of content and a simple interface.

Thanks for the quick replies guys, I really appreciate it. I did a bit of googling and wasn't sure where to look next.

I had been planning on playing Blood for some time now but couldn't decide on buying the disc, purchasing from GoG, or finding some other means (which tbh I have been opposed to doing for quite some time). I'm glad I chose GoG and will probably come back for a few more games later on down the road.

I think for now I'll download your control scheme and try it out (I'm at work now but will be home shortly). I'll let you know later on if it worked out for me or not.

Would your file also work for Blood 2 (also bought through GoG.com)?. I am not an FPS fanatic notmally but I have just finished the first Blood and would like to follow up my "virtual venting" experience with this onde.. However, since I am not really comfortable using WASD keys (they can be a bit awkward), I tried remapping the directional keys via the arrow keys. Problem, when I want to remap "backward" as "down arrow", it keeps remapping the one below it as such instead. Could this be a bug, or am I doing something (or not doing something) wrong?

EDIT: I thought I'd give some additional info after re-reading the post. I ran the GoG installer directly (the yellow icon). the game was saved to c:Program files (x86)/gog.com/blood II. Now forgive my "noobness" but I can't find a blood2.cfg (I don't see the game turning on DosBox beforehand like it did other games I bought through Gog). However I did see a defaults.cfg file, but it wouldn't open via Notepad. Compatibility mode is set to XP sp2. Hope this extra is relevant.

I turned mouse look off as I found it made things more difficult but even so it still doesn't feel right.
So, just wondering what sort of control schemes and sensitivity settings you guys use that you feel work well.
I use WASD for movement, q for actions, e for secondary fire, leftcontrol for crouching, leftshift stays as sprinting, and also I invert mouselook(which is activated by simply pressing u, mouselook that is not inverting thats in the options). I have not used any launcher and the controls work splendidly I have no idea why someone would complain about mouselook being awkward as it works fine for me after doing this.

I honestly don't have the .cfg file anymore, as it was on my old PC. I just re-edit the .cfg file each time I install the game (or any game), and haven't installed it on this system yet. If I get around to installing the game again, I'll just copy-paste the .cfg contents here so I don't have to upload it to one of those sketch file share sites.
