It's nothing devastating at all, but it helps if you know beforehand:
- you can only have 4 HP, not 8.
- I think GoG's version let's you acculmate more than 9 lives (this balances out the less HP-bar somewhat)
- you cannot carry over equipment between levels
- your HP gets fully regenerated when you reload (HP otherwise would carry over between levels). The game only does level-saves, so this isn't infinite HP, but still good to keep in mind in case you pass with1 HP.

I just double-checked the game now and neither of the first two occur. You must have selected "normal game" by mistake and misread the lives display somehow. The other two are not bugs; they're design choices. Many games do the same thing. The only bug I've ever found in Bio Menace is that one purple slug who mysteriously produces no gibs when killed.
