Just got back into the game after a few months and this is what is confronting me. Any ideas on how to fix it?

Do you have Radeon graphics card? I had exact same problem on my 4870, in other Infinity Engine games too, like Icewind Dale or Planescape Torment. It was caused by drivers, and I fixed it by reverting to some older version.
Give other driver versions a try!

Install DDrawFix. That should fix it.

Hickory: Install DDrawFix. That should fix it.
I was having the exact same problem, again on a Radeon.
DDraw fixed it fine.
