After installing the recommended mods (fan patch, townmaps, high-res patch) I started playing just now.

But the game freezes frequently when scrolling or opening the map. I guess it buffers graphics/NPCs or the like because the little loading clock appears in the top right corner. This takes anywhere from 5-20 seconds and appears to be happening more in high density areas.

I'm running the game on Win7 x64, 1920x1200.

Any suggestions?

Uninstall the hi-res patch; it always seems to be more trouble than it's worth. Run the game as normal (which IIRC is windowed mode) and your lag should disappear.

Removing the hi-res patch has almost fixed it. I still get the occasional freeze, but they don't last as long and don't occur as frequently. I'll try another resolution later, maybe 1024x768. The standard one is a little too close to the action.
Glad I could help somewhat. I'd also suggest reading some of the other recent (or recently necro'd) threads for specific technical suggestions you could try. Best of luck to you.
Addendum: Renaming/Removing the ddraw.dll located in the Acarnum folder completely fixed the issue, even widescreen resolutions work fine now.
