Anyone know how to fix this. Been playing with options but not working. I tried Software Mode but its basicly unplayable. How do I get this game to run?

1920x1080 wasnt available in Software Mode. Could this be the issue? Do I need to lower my desktop resolution to play it?

I bought this game thinking it was the Enhanced version (foolish me). Now I cannot even get this old DOS game to run. Frustrated.

Which one Quake you mean, Quake 1?
Q1 here come in two major builds (actually there is few exe files just like for retail 1.09 release) inside one DosBox link and WinQuake+GLquake link. Fisrt one require a bit manual editing, default link lead to game(dosbox) exe while there is specific launcher for it with settings available (resolution, window-mode, output gfx api - try opengl).

For DosBox version chose in launcher 1024x768 resolution and check keep ratio. For GLquake Keep aspect ratio should be enabled in video driver, and chose one of the available resolutions in game. Make sure your GLquake link not have -safemode\software arguments.

If your game currently not started at all, and u need to fix it - you can add -safemode to make it start and reset settings. Restart it without argument and reset again.
There is also iD some minor faults at general gl renders for Q1\Q2. Some resolutions missing.
Look carefully at digits, widescreen resolutions is LOWER variations of normal 4:3.

UPD: and there is 1600x900, so for FHD set 1600x900x32.

According to PCGW for proper widescreen support install one of the sourceport's. Cant say which on are better, another threads have answer to it.

Your solutions sound overly complicated for me. However, reading them did jar loose something I actually knew subconsciously but didnt realize...
