It seems like every other day this game is showing as "Updated", but the last entry in the changelog is from a year ago. What are they changing?

They're updating patch notes on Steam, you can look them up here.

But for example the last update had some balancing changes and it's not described there, you can see them here.

Updates without patch notes are probably on the server side and small bug fixes.
