I have this issue where I'm rescuing HN outlaws from the cannibal pikes, but if I free someone that's unconscious I cannot pick them up, it only gives the kidnap option. I want to pick them up and carry them to a safer location so they can recover. If I leave them there they just get put back on the pikes like 5 seconds later by the cannibals.

Does using the kidnap option in that circumstance incur any relation hit on the victims faction or the victims themselves ?

I have not had an issue with kidnapping anyone causing a reputation reduction with a faction as long as no one from that faction or an allied faction sees you in the act of kidnapping or carrying the kidnapped person around.
