Title says it all

Optional: Favorite Console.
Can't pick just one. Too many games, too many genres, way too many things to take into account!

But I guess I'd go with PS as favorite console.
Shadow of the Colossus. I played it maybe 30 times, and every time I enjoy it as the first time.

Yes *Okay face*
My favourite game of all time is The Legend of Zelda: A Link to the Past. IMHO it's even slightly better than Ocarina of Time.

And my favourite console ... you may guess it: The Super Nintendo Entertainment System aka SNES.

X-Com, and I'm super stoked for the new one coming out.

Civ 2 is on par as my favourite as well.
System Shock 1, absolutely brilliant game.

Well, even then I'd have to qualify it. One game I love is Vampire The Masquerade - Bloodlines. It's one of my favorites, but it's a certain type of setting and doesn't take into account my love for traditional/medieval-type settings and stories.

I can play the Spellforce games over and over, and Drakensang. And Warlords Battlecry 3 is my "I'm buzzed and I don't want to think too much" go-to game, but that's more like a strategy game. I love Mount and Blade, but even the setup for that is quite different than the other games I mentioned. I can't really think of ONE game that gives me everything that I enjoy.
Planescape, Victoria 2, Civ 2, Majesty: The Fantasy Kingdom sim.. please don't make me chose.

Favourite system to game on: PC

Favorute console? That one is a bit harder. I have nostalgic ties to both the NES & SNES, so those two gets an unfair advantage.
Gabriel Knight trilogy! Sorry, I can't pick just one, but if I had to it'd be the first one, Sins of the Fathers.
