I don't have any idea where are you stuck, so I'll just throw this out here in case it may be of help to you: If I remember correctly from when I played the game, sometimes it's not a matter of finding the tiny 'hot spot' with the mouse cursor. Sometimes you have to move Guybrush around to activate an action and/or reveal a new 'hot spot' in the scenery.

Currently trying to get the fish from the bird in the SCUMM bar. Stupid bird. Must be missing something obvious. I have a feeling the troll would be interested in the fish. Not sure though.

My memory of the game is very hazy, to be honest, but maybe my comment is related to your problem after all: I remember there was a pier, and maybe a seagull or a bird or something, and something happens when Guybrush walks all the way to the edge. Something related with a loose plank, maybe?
