The woman is the picture is not allowed to grow up because grown-ups commit sin. So, She thinks she is a little girl. Her mom and dad go out to get her new dolls to play with in the basement. The "dolls" are the dead bodies of college kids who have taken boats out to the island over the years.
It's a total "B" movie, but it stars a legendary, Oscar-winning actor as the Dad, and another legendary actress as the mom. This actress never won an oscar, but she was in the film, "Oscar". She's best known for her role as an iconic, classic TV wife.
