Smannesman: Darn I was looking for the PC DLC buying info, I pre-ordered the 360 version so I know how that works.
IIRC using the link in-game opens up your browser to
http://social.bioware.com/page/me2-dlc where you can select the DLC you want. It'll take you through the process of buying Bioware points. You then download and install the DLC from outside the game.
It's not that difficult to do, but paying in Bioware points can be annoying. Why they couldn't just attach a price to each item instead points... well, I guess they like the model in which you end up with some points left over >_<

In the case of someone in Japan, all you needed to do was have someone in the US buy and email the key. Then activate via a US based VPN. So for those outside the US, as the key registers as "Mafia 2 Amazon Retail" you may or may not need a US VPN to add the key to Steam.
That depends on whether Valve are just applying that IP block to us or not U_U
