The codes that represent $5 in promotional credits you got for buying any eligible game from Amazon since I think the latter half of November. You should have received an extra e-mail with each purchase containing the code.

What i read from Amazon reviews(most of them is 1-star rating) the Windows Live and SecuRom in Gta4 giving nightmares to the players. Plus blaming GTA4 for an unoptimized bad console port. So much 1-star rating. I haven't looked the other ones yet. Buti don't know about the GG version.
